My Story: Maria Marte, WellCare Member
Maria Marte, 69, has diabetes, suffers from heart disease and was experiencing kidney failure. Maria was deteriorating at home but was hesitant to get needed life-saving dialysis treatment. With support from her family, medical team and a WellCare field-care management nurse, Maria is getting the assistance and supplies she needs, including oxygen, ambulance transportation and a paramedic crew to help transport her from her walk-up apartment to dialysis treatments three times a week.
My Story: Yolanda Flahive, ‘Ohana Member
Yolanda Flahive’s dementia was worsening, and she was no longer able to care for herself. Her daughter, Debra, was running out of options to provide a safe environment for her mother to live. ‘Ohana’s case management team stepped in and worked closely with Yolanda’s family to find a solution through ‘Ohana’s partnership with the state of Hawaii’s adult foster care program. Yolanda is now receiving nursing-home level care in a home environment, and is safe and happy.
My Story: Jeremy Lucas, WellCare Member
Jeremy Lucas was 30 years old when he suffered a traumatic brain injury and lost a leg as a result of a motorcycle accident. His mother Amelia quit working because she wanted to care for him at home. She did not expect it to be so difficult to coordinate the supplies and services Jeremy needed. With the help of a WellCare social worker – who was determined to get him the right care, in the right time, in the right setting – Amelia was able to get the things she needed to help Jeremy live the best quality of life possible.
My Story: Gloria Owens, WellCare Member
Gloria Owens knew she needed help, but did not know how to get it. The 72-year-old had recently had her leg amputated due to circulation problems and was coping with a host of other medical issues, including depression, when she started working with her WellCare case manager. The social worker knew exactly how to help Gloria manage her chronic conditions. What the social worker did not expect was how their special relationship would help Gloria develop a new appreciation for herself and life.
My Story: Brandi Logan, WellCare Member
Brandi Logan is deaf and lacked a support system to help her properly care for her 3-year-old son Dylan, who was experiencing developmental delays. With the help of a WellCare social worker, Brandi was connected to needed medical care, as well as social services to help her pay medical bills, access transportation to doctor appointments and manage daily tasks. Dylan was enrolled in school and received the physical, occupational and speech therapy he needed to thrive.
